Summary
This article explains how to conduct Instant Searches by Individual Name. A First and Last Name must be supplied to complete the search. Search results return potential matches for Federal Exclusions and State Exclusions (aka Sanctions) as well as OFAC sanctions lists. Results are flagged with a red Needs Review status and can include details such as first, middle, and last name, address, exclusion and reinstatement dates, and notes from the exclusion source. If no matches are found, the system displays No results found.
Overview
Organizations contracted for Instant Searches can conduct searches using an individual’s first and last name to identify potential matches across federal and state exclusion lists, as well as OFAC sanctions lists.
Available Name-Based Matches
When searching by individual name, the sources available include:
- Federal and State Exclusions (aka Sanctions)
OFAC (Specially Designated Nationals (SDNs), Blocked Persons, and Consolidated Sanctions Lists)
How to Search by Individual Name
Enter the first and last name of the individual in the search box.
The system will return any potential matches across the available sources.
Individual Name Result Types
Federal Exclusions & State Sanctions
If a federal exclusion or state sanction is found with a name match or a similar name, it will be indicated with a red Needs Review status. If there are multiple exclusions or sanctions for federal or state sources, they will be listed as individual issues. If an NPI is provided at the exclusion source, it will display beneath the NPI column for easy review.
More details for each potential exclusion can be accessed by clicking on the individual result.
Once a result is selected, details from the source can be accessed by clicking the arrow on the right side of the result.
Details from the exclusion source will be provided and can include the following:
First Name
Middle Name
Last Name
Address
Type
Start Date
Excluded Date
Exclusion Notes

No Results Found
If there are no results that match the name of the individual searched, the result will display No results found.
OFAC
If an OFAC result is returned during an Individual Name search, it will be displayed with a red Needs Review status. OFAC results are classified under Other Exclusions. Additional information regarding the OFAC result can be viewed by selecting the individual result.
Once a result is selected, details from the source can be accessed by clicking the arrow on the right side of the result.
