This article explains how to access historical primary source license verifications in ProviderTrust by locating an individual in Population, opening License History, selecting the applicable license type and date, and downloading a PDF copy of the verification report.

Accessing Historical Primary Source License Verifications

ProviderTrust offers a Historical License Verification report that allows you to access past primary source license verifications for credentials previously verified in the system on a selected date.

To view a historical license verification:

1. Navigate to Population and select the individual whose license history you would like to review.
2. Click the individual’s name, then select License History.

From the License History page, you can:

• Toggle between the different license types currently in monitoring
• Select the specific date for which you would like to download the primary source verification screenshot

Click Download to automatically generate a PDF copy of the primary source verification report.

The PDF includes:

• The report Date Generated
• The user who generated the report
• The organization name
• The Date Searched at the primary source
• The provider’s information 
• Primary Source board screenshot of the credential
