How do I log in to ProviderTrust?

What is my Username?
Your username will always be your email address.
What is my Password?
Your password should have been set up the first time you logged in to ProviderTrust. As a reminder, your password must meet the following requirements:
- Be at least 10 characters long

What if I forgot my Password?
If you forget your password, select the text, Click here if you have forgotten your password. This will direct you to enter your username (email address) to reset your password securely. You will receive an email with detailed instructions on resetting your password.
Oh no! I was locked out of my account!
If your login information is incorrect, you will receive a message stating Login failed. Please try again. You have 10 attempts to login before your account becomes locked.
