Summary:
ProviderTrust displays license board actions identified during the verification process when available from the primary source. These alerts appear within the Alerts tab and are also included in the Individual Audit Report and Configurable Reports generated from the Alerts tab. Full action details can be viewed directly on the corresponding state licensing board’s website.

Overview
When ProviderTrust identifies a license board action associated with a monitored provider’s license during verification, this information is displayed directly within the Alerts tab.
Board action data is sourced from the primary licensing authority, ensuring the information you see reflects official regulatory records.
License board action information will display in multiple locations across your ProviderTrust platform and reports:
1. Alerts Tab
Within the Alerts tab, when selecting a license alert, the presence of a license board action is indicated in the “From the Primary Source” section.
Next to Alert Reason, you will see the message:
“There is a board action present.”
To review the full details of a license board action, continue scrolling downward on the From the Primary Source card.
Note:
Clicking on the Primary Board Action Screenshot image and the Document Screenshot link will expand the view. Additionally, screenshots captured from the primary source as well as Board Action Data will vary.
2. Configurable Reports (Generated from the Alerts Tab)
When generating configurable reports from the Alerts tab, board action details—when available—will appear in the following columns:
Verified Alert Reason(s)
Verified License Action
Verified License Details
These columns provide a snapshot of the latest verified license information, including the existence of any board actions.

3. Individual Audit Report
If a provider’s license includes a board action, this information is also reflected in the Individual Audit Report.
The report will show the Alert Reason (Board Actions) and the Source (i.e. License Issuer) for audit and documentation purposes.
